Athletic Forms & Policies
Students and parents are required to review and comply with the forms below in order for the student to participate in athletics.
The NCAA Guide for the College-Bound Student Athlete answers important questions for groups of people involved in the NCAA initial-eligibility process, including high school students who hope to compete in college sports an NCAA Division I or II school, parents, guardians and family members of high school students, high school counselors and athletic administrators as well as high school and nonscholastic coaches.
PIAA Physical Form
Prior to any student participating in practices, inter-school practices, scrimmages, and/or contests, at any PIAA member school in any year, the student is required to complete a comprehensive initial pre-participation physical evaluation (CIPPE) and have the appropriate person(s) complete the first six sections of the CIPPE Form.
Parent/Coach Communication Plan
The purpose of the parent/coach communication plan is to establish a mutual respect between parents and coaches and provide guidelines for parent/guardian and parent/coach communications.
Hazing violates the purpose and mission of Catholic education, wherein students are called to grow in their commitment to God, one another, the Church, and the wider community.
UPMC Sports Medicine
Central Catholic and UPMC Sports Medicine have established a contractual agreement to provide a Certified Athletic Trainer to aide in the prevention, recognition, evaluation, and treatment of athletic injuries.
Coach and Moderator Clearances
The following are links to websites for clearances that are mandated by the Commonwealth of Pennsylvania for all paid and/or volunteer employees working with minors. Additional information can be obtained through Office C.
