PIAA (CIPPE) Forms
All student-athletes wishing to participate in a PIAA sanctioned sport must complete and submit the PIAA CIPPE form. If a student-athlete does not have this form submitted he is NOT allowed to participate in any athletic activity until all forms are submitted.
All pages of the packet must be turned in for a physical packet to be considered complete. PIAA CIPPE forms can be turned in one of two ways. Student-athletes or their parents can submit the forms to Office A or the packet may be turned into the Athletic Training Room directly. PIAA CIPPE packets should not be given to your coaches.
Fall sports season athletes must have a PIAA CIPPE form turned in order to participate a fall sport. If your son will be playing a sport in any subsequent sport seasons he must submitted the Section 7 of the PIAA CIPPE packet. This page titled “Parent-Guardian Re-Certification.” If you answered “Yes” to any of the questions on the Section 7, Parent Re-Certification," you must also submit the Section 8, “Doctor Medical Re-Certification” Form. These forms can also be submitted to the Assistant Principal for Student Affairs (Office C) or the packet may be turned into the Athletic Training Room directly.
If you are having issues scheduling a doctors’ appointment for your sons PIAA CIPPE sports physical, there are many UPMC Urgent Care and MedExpress, or pharmacies located in the area that can perform the physical exam at a low cost and do not involve your insurance.
Beginning with the 2016-2017 school year, all PIAA physicals and re-certification forms will move to a paperless version. This paperless version is created through the website “Family ID."
If you are a first time user you will be prompted to create an account using your email and password. If you are a returning user, simply login and begin the registration process. While registering via Family ID you will be prompted to upload a copy of the PIAA Section 6. This page is where your sons’ doctor will clear him to play sports. This page can be uploaded to Family ID as a .pdf or .jpeg file. If you do not have the access to upload the Section 6 electronically, the page can be submitted to either the Office of the Assistant Principle for Student Affairs (Office C) or directly to the athletic trainer, whose office is located in the coaches’ pavilion.
